Independent fee‑for‑service provider. Commercial insurance, liability, and workers’ compensation are billed directly for covered services.
Assistant‑at‑surgery services have federal billing rules. Medicare/Medicaid/TRICARE may impose statutory limits on which provider types may be reimbursed or billed. Patients are not billed for government‑restricted assistant‑at‑surgery services.
Self-pay arrangements may be available for services that are not covered by insurance and are not restricted under federal or state payer regulations. Self-pay services are considered on a case-by-case basis and must be arranged in advance of services being rendered and approved by the facility when applicable.
HIPAA‑compliant; operative documentation aligned with facility and payer standards; correct coding/modifiers applied as applicable.
